What the inspector wrote

On 4/28/2025 @ 8:35AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced inspection in reference to an allegation that staff are not performing emergency evacuation drill. LPA interviewed 6 staff today. Staff interviewed today indicated that the last drill conducted was Summer of 2024. Infant staff interviewed today also stated that the facility has not conducted a fire/evacuation drill this year. It was also alleged that staff are not moving infants to cribs after falling asleep. Staff interviewed today indicated that there is a child in the room having difficulties sleeping in the crib. Child wakes up as soon as child is placed on the crib. Staff has allowed child to sleep on the floor to rest. The allegations are valid because the preponderance of the evidence have been met, therefore, the above allegations are found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter number 1) the deficiencies are being cited on the attached LIC 9099D. The Notice of Site Visit was provided, and LPA observed posting. Licensee is advised it must remain posted for 30 days. Exit interview conducted and report was reviewed with Megan Eckert, Site Director. Appeal rights were also given.

  • INFANT CARE ACTIVITIES

    101430 INFANT CARE ACTIVITIES(a)(3)(E) If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible.This requirement was not met as evidenced by: Based on staff interviewed, a 5-month old infant was allowed to sleep on the floor(daily) due to the infant's difficulty in sleeping. According to staff the infant will awake as soon as they try to lay infant in the crib.

    101238(c) BUILDINGS AND GROUNDS.(c) All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard shall be kept free of obstruction.This requirement was not met as evidenced by: Based on observation, a car seat and a back pack were observed stored near the exit. During the recent earthquake, staff had to remove a couple of car seats out of the way before they safely got out of the building.
