What the inspector wrote

On 4/28/2025, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced case management inspection in reference to a self-reported incident involving an infant who hit the top of his right eyebrow on the table causing a cut. Child was taken to emergency and received 3 stitches on the eyebrow. LPA did not observed tripping hazards around the table. Incident occurred while children were at play. Staff provided immediate attention to injured child. LPA met and toured the infant room with Megan Eckert, Site Director. Observed present today were 13 infants with staff Rocio Loarca, Melissa Armenta, Alicia Ruiz, Carly Rivers and Rosa Rubio. Also included in today's discussion was the infant room's evacuation path. During the recent California earthquake (April 14th), the staff experienced a challenge in evacuating the infants out of the classroom. According to stff, there were 2 car seats blocking the exit out of the building. Staff had to remove the car seats out of the way so that the evacuation crib (with infants) can fit through the path. Staff were able to exit out of the building safely. TYPE B DEFICIENCY WAS CITED TODAY. Type B deficiency if not corrected poses a potential risk to the health, safety or personal rights of clients in care. Exit interview was conducted with Ms. Eckert. A copy of this report was reviewed and provided. Appeal rights was also given. Notice of site visit must be posted for 30 days. See LIC809D for deficiency.

    101430 INFANT CARE ACTIVITIES(a)(3)(E) If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible.This requirement was not met as evidenced by: Based on staff interviewed, a 5-month old infant was allowed to sleep on the floor(daily) due to the infant's difficulty in sleeping. According to staff the infant will awake as soon as they try to lay infant in the crib.

    101238(c) BUILDINGS AND GROUNDS.(c) All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard shall be kept free of obstruction.This requirement was not met as evidenced by: Based on observation, a car seat and a back pack were observed stored near the exit. During the recent earthquake, staff had to remove a couple of car seats out of the way before they safely got out of the building.
