What the inspector wrote

On 5/8/2025 @ 8:40AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced inspection in reference to the deficiencies cited on 4/28/2025. Observed present today were 14 infants (mobile and non-mobile) with staff R. Rubio, M. Armenta, R. Loarca, C. Rivera & A. Ruiz. LPA Diaz met and toured the infant room with Ass't Director Elizabeth Saenz. Facility's plan of correction to sleeping the infant was to place child on a mat to nap in the play area. Plan of correction submitted on 5/5/2025 indicated that an Evacuation drill was conducted on 4/29/2025. Type B deficiencies were cited today. Civil penalty was also assessed for failure to correct. Exit interview was conducted with Elizabeth Saenz. LPA reviewed and provided a copy of the licensing report. Appeal rights were also given. Notice of Site Visit must be posted for 30 days.
