On 11/3/23 at 10:30am, Licensing Program Analyst (LPA) Patrick Ma, visited the facility to conduct a case management site inspection. The purpose of this visit is to follow up on a self-reported incident that occurred on 10/12/23. Upon arrival, LPA met with Assistant Principal, Michelle Austen. Present at the facility were 59 daycare children and 8 staff in 6 rooms. Proper supervision and ratios were observed. Facility reported on 10/12/23 Child C1 was discovered by staff S3 unsupervised by the playground doorway. LPA conducted interviews with staff, reviewed related documents, and made a confidential names list. Based on information gathered, child C1 was left on the playground and unsupervised for approximately 20 minutes until discovered by another staff after C1's class returned to their classroom. Staff supervising C1's class was found not following school's face-to-name policy/procedure and were subsequently terminated from employment. See 809D for deficiency cited. LPA Patrick Ma informed Assistant Principal Michelle Austen that this report dated 11/3/23 documents one Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Ma informed the facility representative to provide a copy of this licensing report dated 11/3/23 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative Michelle Austen. A notice of site visit was given and must remain posted for 30 days.
