Based upon Director statement and name to face documentation from the facility app, C1 was left unsupervised on the playground for approximately 1-3 minutes which is a potential health, safety and personal rights risk to children in care.

Based upon interviews conducted with reporting party, staff members and parents of enrolled children, about once a month since October 2024 there have been 13 children with one staff member which is a potential health, safety and personal rights risk to children in care.

This requirement was not met as evidenced by: Based on interviews and documentation, it was determined on at least 3 separate occasions, staff S1 violated children’s personal rights by grabbing their arms and swinging them in another direction, yanking them, and exhibiting other rough behaviors toward children which poses/posed an immediate health, safety or personal rights risk to children in care.

    101229(a)(1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: Based on investigation interviews and documentation children in the 2’s classroom have been left unsupervised in situations when a single staff is using the diaper changing table and unproperly supervised using the bathroom while they are potty training. Also, documentation determined child C1 was left unsupervised in a classroom and discovered by a staff which posed a potential health, safety or personal rights risk to children in care.

    101223(a)(1)&(3) Personal Rights: The licensee shall ensure that each child is accorded the following personal rights (1)…dignity in his/her personal relationships with staff and other persons…(3) To be free from corporal punishment or unusual punishment, infliction of pain…This requirement was not met as evidenced by: Based on interviews and documentation reviewed it is determined, staff S1 has spoken inappropriately, yelled at, or used an unnecessary forceful tone as well as grabbed, yanked or other rough behaviors with children in care as a means of discipline and/or redirection which posed an immediate health, safety and/or personal rights risk to children in care.
