What the inspector wrote

On 11/8/24 at 9:10AM, LPA Patrick Ma made a case management-deficiency visit to conduct investigation for information received during another visit. Upon entry, LPA met with Office Administrator, Brandi Christman and explained purpose of the visit. Director was not present during visit. During this visit LPA interviewed staff and children and reviewed relevant documents. Based on information from interviews and documents that was brought to light for a different purpose, it was determined on at least 3 separate occasions, staff S1 violated children\342\200\231s personal rights by grabbing their arms and swinging them in another direction, yanking them, and exhibiting other rough behaviors toward children as a means of discipline and/or redirection. See LIC 809D for deficiency cited. LPA Ma informed facility representative Brandi Christman that this report dated 11/8/24 documents 1 Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Ma informed the facility representative to provide a copy of this licensing report dated 11/8/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative Brandi Christman. A notice of site visit was given and must remain posted for 30 days.
