What the inspector wrote

On 1/12/26, Licensing Program Analyst (LPA) Saraliz Velando made an unannounced case management visit to follow up on a self-reported incident that occurred on 12/16/25 wherein a child was left unattended by staff during a transitioning period. There were 10 children with 2 fully qualified staff at the time of the incident in the Preschool 3 classroom. Child #1 was left outside of the classroom with the door closed after transitioning. Staff #1 conducted the transition of bringing the children back to classroom after outside play. Staff #2 conducted the 3 rd part of the transition check and noticed they were short one child. The child was found right outside the door of the classroom. The parent was notified by the Director about the incident and it was also reported to Licensing. LPA interviewed the Director, interviewed staff members, and collected pertinent documentation. After thorough investigation, a TYPE B deficiency was issued on the attached LIC 809-D . Exit interview was conducted with the Director, Letisia Ford. A copy of this report and the Appeal Rights was provided. A notice of site visit was posted and must remain for 30 days.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision - (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced by: Based on Director and staff interviews the licensee did not comply with the section cited above in that Child #1 was left unattended for approximately under one minute without staff supervision which poses/posed a potential health, safety or personal rights risk to persons in care.
