What the inspector wrote

On date and time listed, Licensing Program Analysts (LPAs) Ana Noble and Nasha King arrived at the facility to conduct an annual inspection as part of a compliance review. The Licensee has applied to increase the capacity at the Child Care Center from 70 to 104. Fire clearance was granted on 09/13/2021. LPAs toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Normal days and hours of operation will be: Monday-Sunday 5:30 am - 11:00 pm A review of the staff records and a sampling of the children's records was conducted as part of this evaluation. The licensee is asked to update the following documents, if applicable, and submit to licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) 4. LIC 309 Administrative Organization (only if changes have been made) 5. LIC 308 Designation of Administrative Responsibility (only if changes have been made) \302\267 The following items have been posted and are updated where necessary: - License - Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent\342\200\231s Rights Poster (PUB393) - Personal Rights (LIC613A) - Child Car Seat Law - Menu \302\267 The facility is operating within the terms of the license \302\267 Ratios were met during this inspection \302\267 Appropriate supervision was provided during this inspection \302\267 Classrooms are equipped with age appropriate furniture and equipment in good condition \302\267 Classrooms are clean and free of hazards \302\267 No weapons stored at the facility as of 10/18/2021 \302\267 There are no bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Medications are stored where inaccessible to children \302\267 Hazards are stored where inaccessible to children which include: disinfectants, cleaning solutions and other items that are dangerous to children \302\267 All floors shall be clean and safe- This requirement was not met- in Rooms 1 and 2, there were multiple rips and tears in the carpets, which can be a tripping hazard. Additionally, the carpet in Room 3 showed signs of wear and tear and had a big, dark and dirty stain on it. \302\267 Toxins are locked \302\267 Bathrooms were observed to be safe, sanitary and in operating condition-This requirement was not met: upon LPAs entering one of the children's restroom, LPAs could smell urine even while wearing a mask. \302\267 Playgrounds are enclosed by appropriate fences and free of hazards- This requirement was not met: LPAs observed rusted and disrepaired toys on the playground, and the sitting tables had paint chips and rotted wood \302\267 Outdoor activity areas are supplied with age and size appropriate equipment in good condition- This requirement was not met: play structures were rusted \302\267 Food preparation area is clean and free of vermin \302\267 Food is stored appropriately and protected from contamination \302\267 All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair \302\267 Uncontaminated drinking water shall be readily available both indoors and out and is provided by a water dispenser \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall- This requirement was not met-LPAs observed that there was no cushion or other materials to absorb the children's falls \302\267 Menus shall be posted at least one week in advance in a place visible by the child\342\200\231s authorized representative, dated and kept on file for 30 days, and made available upon request. \302\267 Sign in/Sign out record was reviewed and meets regulation requirements \302\267 A Staff member is present with current Pediatric CPR/First Aid which expires on 02/2022 \302\267 Opening and closing staff member\342\200\231s CPR/First Aid expires on 02/2022 \302\267 Director completed Health and Safety Training 10/12/2014 \302\267 Each Staff\342\200\231s files contain the required health screening as specified in section 101216(g) \302\267 Each child\342\200\231s file contains the required identification and emergency information and medical assessment \302\267 A review of staff records on 10/18/2021 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov \302\267 Facility is providing IMS This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. School-Age Indoor Activity Areas LPA has determined that there is sufficient space to accommodate 104 children. Licensee will request for a waiver to use the additional sink and toilets in the preschool classroom, while preschool children are not present (currently the facility is not enrolling preschool children). Room #6 23.42 x 29.58= 692.7636 Room #7 32.42 x 23.25= 753.765 Total = 1446.5286= 41 Children School-age Bathroom Fixtures 5 toilets/2 urinals x 15 = 105 children 5 sinks x 15 = 75 children (Preschool classroom has 4 toilets and 4 sinks, waiver will be requested to use when no preschooler are present or enrolled) School-Age Outdoor Activity Area : LPA has determined that there is sufficient space to accommodate 104 children. Limiting factor for school-age capacity is sinks. School-age capacity is limited to 75 (due to sinks), However the Licensee will request a waiver to use the 4 of the preschools toilets and sinks, while no preschooler are present or enrolled (Capacity will need to be decrease if preschooler are enrolled). An exit interview was conducted and during the interview, the Director, Claudia Garcia confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address. A NOTICE OF SITE VISIT WAS ISSUED and MUST BE POSTED IN PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. Appeal Rights were discussed along with a copy of this report were provided to the Licensee. This report must be available for review, upon request, for the next 3 years.

The ground turf was coming up, which could cause a tripping hazard. Additionally, LPAs observed multiple holes in the ground. This poses a potenial risk to the health and safety children in care if not corrected.
