LPAs could smell the urine through mask use. This poses a potenial risk to the health and safety children in care if not corrected.

Additionally, the carpet in Room 3 showed signs of wear and tear and had a big, dark and dirty stain on it. This poses a potenial risk to the health and safety children in care if not corrected.

The ground turf was coming up, which could cause a tripping hazard. Additionally, LPAs observed multiple holes in the ground. This poses a potenial risk to the health and safety children in care if not corrected.
