What the inspector wrote

On July 13, 2022 at 10:15 a.m. Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced inspection. Upon arrival LPA met with Director Cheryl Gillins and proceeded to tour the facility. There were 7 children present with 3 staff members. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility. LPA observed that the director altered the infant facility by removing the barrier that separated the infant nap area from the activity space. The director did not notify Community Care Licensing or obtain approval from the department before the alteration was made. See LIC809D for cited deficiency. An exit interview was conducted with Director Gillins and appeal rights (LIC 9058 1/16) were discussed. A copy of this report as well as a copy of the appeal rights were given to Director Gillins. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post Notice of Site Visit.

This requirement was not met as evidenced by: Based on a record review, on 4/18/22 five children were allowed to remain at the facility with symptoms of illness. This poses a potential health and safety risk to the children in care.

This requirement was not met as evidenced by: Based on LPA's observation the infant facility/nap area was altered prior to notification and approval by Community Care Licensing. This poses a potential health and safety risk to children in care.
