What the inspector wrote

On 10/2/25 Licensing Program Analyst (LPA) Bailey Bowman conducted an unannounced inspection. Upon arrival LPA met with Director, Bailey Bowman. There were 1 infants present with 2 Infant staff. See LIC809-D for deficiency cited. Based on file review and parent interviews for a different visit, there was corroborating evidence that multiple injuries to an infant were not reported to the Department as required by Title 22 regulations. An exit interview was conducted with Director Bailey Bowman and a copy of this report was provided. A notice of site visit was posted and must remain for 30 days.
