Follow-up on corrections·December 7, 2022

On 12/07/22 at 1:55pm, Licensing Program Analyst (LPA), Selina Siao conducted an unannounced case management inspection. Upon arrival, LPA conducted a tour to gather census. LPA observed the following ratios: Giraffe Room had 15 children napping supervised by staff Gema Garcia Castillo. Panda Class had 16 children mostly napping on mats and Koala Room had 7 children napping and both rooms were assigned to be supervised by staff Fataneh Mazdeh aka Ms. Tina. Tiger Room (toddler class) had 10 children with some napping and some awake supervised by teacher Ledy Rodriguez and staff Rosa Lorenzo. Wild Cats Room (toddler class) had 12 children napping supervised by staff Maria Ramirez. All staff members have the required background clearances and are associated to the facility. During the inspection, one of the child in the Panda class walk to staff Fataneh Mazdeh aka Ms. Tina to inform the staff that he needs to use the restroom. Staff informed the child to go ahead to use the restroom. The child in the restroom and the seven children in the Koala class were left without visual supervision during the tour as there was only one staff assigned to supervise the two classrooms. Director stated that one of the staff had to leave as she was feeling sick and had to go to the doctor due to an emergency situation. LPA later met with the Director who was in the infant room with facility# 376701141 (Cuddle Bear) along with staff Carmen Ortega Torres supervising eight(8) infants.
