Based on LPA's observations, interviews conducted, and records reviewed. There was no corroborating evidence regarding the allegation. Due to conflicting information obtained from the interviews, and although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Therefore the above allegation is found to be unsubstantiated. A Notice of Site Visit (LIC 9213) was provided to the Director and advised must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Licensee was advised failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Licensee it being posted in a visible area. An exit interview was conducted and the report was reviewed with the Director, Claudia Huerta. No deficiencies cited.
