What the inspector wrote

On 12/29/2025, at 2:30 PM, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced Case Management inspection to follow up on an Unusual Incident reported to the Department on 12/04/2025. Upon arrival, LPA met with Director, Krizia Beltran, and discussed the purpose of the inspection. LPA was led on a tour of the facility. There were 18 infants present and 6 staff during the inspection. LPA interviewed Director and staff, and reviewed video footage of the incident. Based on interviews conducted and review of video footage, it was determined that on 12/03/2025 around 3:26 PM, Child 1(C1), was given a cup of breast milk to drink while seated on the floor. Child 2 (C2) who was seated on the floor nearby, crawled up to C1, grabbed the cup from C1's hands and proceeded to drink a sip from the cup before Staff 1(S1) intervened. Director reported the incident immediately to the parents of children involved and the incident was reported timely to Licensing on 12/04/2025 via an Unusual Incident Report (LIC 624.) Per California Code of Regulations, (Title 22, division 12 & Chapter 1) one (1) Type B citation is being cited on the attached LIC 809-D. An exit interview was conducted with Director and a copy of this report along with the Appeals Rights (LIC 9058) was provided. Notice of Site Visit must be posted for 30 days.
