On Aug 06, 2025 at approximately 9:05 AM, Licensing Program Analyst (LPA) Tso conducted an unannounced, case management visit. LPA met with Director, Michie Minamoto, and explained the purpose of the visit. Present in the facility is director, 2 staff, and 0 children in care. The school year will start from 8/13/20205. The case management visit is regarding an unusual incident that occurred on June 13, 2025. Facility self-reported incident to CCLD on June 13, 2025. On June 13, 2025, at approximately 4.10 PM, the children were playing at the nearby Lincoln Park, lower structure area. C1 and C2 were running to at reverse direction around a play structure. They ran into each other, not aware that there was someone coming from the other side. They collided and bumped their forehead. C1 was wearing eye-glasses, and the frame of C1\342\200\231s glasses cut into C1\342\200\231s eyebrow. A teacher sterilized the wound of C1 with antiseptic wipe to stop bleeding. At the same time, the other teacher took care of C2 and checked and confirmed that there were no visible injuries on C2. Both parents of C1 and C2 were informed to pick them up. C1\342\200\231s parent informed the Director that C1 would be taken to the emergency room immediately. LPA talked to the Director. The Director stated that both C1 and C2 had not been absent from the school and returned to the school on the coming Monday, 6/16/2025. Per the Director, both C1 and C2 were doing well when returned to school. There was not sufficient evidence for any violations. LPA reminded the Director to pay for the license fee and informed the Director the PIN for fee payment. There were no deficiencies cited at this time under CCR, Title 22, Div. 12, Chapter 3. A copy of today\342\200\231s report was given to the Director. Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Michie Minamoto.
