What the inspector wrote
On February 19, 2026, Licensing Program Analyst (LPA) Leong arrived at the facility to conduct an unannounced annual inspection. LPA met with the Director, Michie Minamoto, and explained the purpose of the inspection.
At the time of inspection, the facility was staffed by the director and 7 staff members, with 25 preschool-aged children in care. The facility was operating within the licensed capacity and staff-to-child ratio requirements. LPA verified that background check clearances from the California Department of Social Services (CDSS) had been granted to the director and 7 staff members.
The facility operates in two rooms, which are clean, well-ventilated, and maintained at a comfortable temperature. Age-appropriate toys, materials, and furniture were available and in good condition. All poisons, cleaning solutions, and hazardous materials were stored in areas inaccessible to children. The bathroom was clean and equipped with sufficient toileting supplies.
Three children\342\200\231s Epi Pens were observed to be current, in the original packaging, and inaccessible to children.
The facility was equipped with fire extinguishers, fully stocked first-aid kits, carbon monoxide detectors, and smoke detectors.
Lunch and snacks are provided by the facility. Lunches are prepared by an outside vendor and delivered in the morning. Packaged snacks are stored on-site, and fruits are prepared on-site. Menus were posted on the wall and available for review. Paper plates and cups are available for the children to use. The children bring their own water bottles for refilling, and the water comes from a filtered sink.

The children nap on cots, and additional cots were observed in the facility. Parents provide blankets and sheets that are laundered weekly.
The outdoor play area was fully fenced and equipped with age-appropriate toys and equipment. Toys and equipment appeared in good condition. The facility\342\200\231s children also use the Lincoln Park Playground as their outdoor play area. The City of San Francisco requires a permit to use Lincoln Park, which was reviewed and found to be current. No pools, spas, or other bodies of water were observed on the premises.
Required licensing documents were posted and available for review. Emergency disaster drills are conducted and documented. The most recent drill was on February 10, 2026. Children are signed in and out on an electronic app called Brighwheel. The app confirmed that all 25 children were signed in to the facility.
LPA reviewed five children's records and found that each child\342\200\231s record had the required licensing forms.
LPA reviewed the records of five staff members, and one holds a valid Pediatric CPR/First Aid certification. The facility is only required to have one staff member on site to be certified in CPR and First Aid. All five staff members' transcripts were reviewed and found to have the educational requirements to be a preschool teacher.
Two staff files were found to be missing measles immunization records and contained incomplete LIC 503 forms, resulting in two technical violations.
Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
