What the inspector wrote

On August 14, 2023., Licensing Program Analyst (LPA) Van conducted an unannounced inspection to finalize this complaint and deliver the findings to the Center. LPA met with the Site Supervisor, Xiao Ping Qiu. The purpose of the inspection was explained, and entry to the Center was granted. Eight staff supervised 27 children today, 12 toddlers aged 18 – 36 months and 15 infants under 24 months. The teachers and children ratio were met. During the inspection, the Center operation personnel, Stacey Toledo, arrived. As part of the investigation, LPA interviewed staff members, conducted surveillance visits at Noe Vally Court Park, and reviewed the facility admission agreement/parent handbook. Based on the information obtained on the date in question, the police were contacted. A wellness check by the police officers determined that the child was physically unharmed and had no sign of abuse. Per the staff involved, the police officers also offered to give the staff and the children a ride back to the facility. The reporting party was asked to confirm the event, but LPA never received a response. Based on the record review, parents know that the facility utilizes the public parks for outdoor space usage. In addition, parents need to complete a field trip consent acknowledgment, and the facility also informs parents of each upcoming field trip. Parents are invited to go on these trips to provide increased supervision and adult/child ratios. The Department has investigated the above allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Therefore, the allegations are Unsubstantiated. An exit interview was conducted with the Site Supervisor. A copy of the report was provided to the Site Supervisor after the inspection. LPA informed the Site Supervisor that the site visit notice must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
