What the inspector wrote

On April 24, 2026, at 2:00pm, Licensing Program Manager (LPM), Garfield Leung and Licensing Program Analysts (LPA), Almond Tso (certified bilingual Chinese - Cantonese) met with Licensee, Miao Yin Zhou for an Office Meeting due to a serious violation with facility number 384004524. The meeting was conducted in Cantonese. Discussed during the meeting were the following: 1. On 3/16/2026, at approximately 10.15 am, an 18-month-old child in care was left unattended at a public park when the licensee and facility staff transited the other children in care back to the facility. The regulations of Lack of Supervision were violated. LPM reminded the Licensee that leaving a child unattended in a public park would impose immediate and serious health and safety risks on the child. 2. Per the Licensee, the child was dropped off to the Licensee on the way to the public park, so that it would be easily to forget that child(ren) as the buggy(ies) passengers were same as those of leaving the facility when return to the facility. 3. LPM recommended the followings for the Licensee to prevent the reoccurrence of the violation mentioned at Point 1 above. - LPM reminded the Licensee to have accurate communication among the helpers and Licensee to ensure the number of children inside the facility and at the public areas to ensure the children in care not left the facility, nor any children left at the public areas without the knowledge of the Licensee and / or helpers. (Continued on page 2, ...) (Continued, page 2, ...) - Prepare the count sheet to do the head count for the children in care before leaving the facility, arrive at the public areas such as a park, before leaving the public areas, and arrive at the facility. - If there were 2 or more care providers, 2 providers should do the head count for the children in care separately. Both providers should sign the head count sheet. The head count is recommended to be retained for two weeks. - The Licensee may consider preparing the vests with the facility name and contact number for easy identifying the children in care. LPM also explained that the Department’s goal is to work with the Licensee to gain compliance. Future noncompliance and further citations may lead to administrative action against the license. More frequent visits would be conducted. >>> A copy of this report was discussed and printed for Licensee whose signature on this form confirms receipt of the report . This report must be available in the facility for public review.
