What the inspector wrote

On January 18, 2023 at approximately 8:30am, Licensing Program Analyst (LPA) Catrina Quimbo conducted a preliminary pre-licensing inspection. LPA met with applicant, Kristina Gonzalez, and director, Kimberly Quinonez. The applicant is requesting for a combination center of a preschool application with a toddler option component. The applicant is requesting a preschool capacity of 30 children (2.5 years to entry into first grade) and a toddler option capacity of 15 children (18 months to 36 months). Hours of operation will be 8:30am to 5:00pm. The facility director will be Kimberly Quinonez. Applicant plans to utilize electronic signatures for families to sign children into and out of facility. LPA measured space indoors for preliminary capacity and inspected outdoor area applicant intends to use for an outdoor area. Indoors: The facility contains two classrooms: preschool and toddler classroom. The preschool classroom measures for 1,221.32 sq ft. allowing for a capacity of 34 children. The toddler classroom measures for 659.15 sq ft. allowing for a capacity of 18 children. Applicant is aware the toddler option component cannot exceed 12 children. The total square footage equals 1,880.47 sq ft. allowing for a capacity of 53 children. Applicant is aware preschool children and toddler option children must be kept physically separate. The facility also contains a director's office and multiple staff lounges. Second floor of facility will be used for staff purposes only and will not be utilized for children. Facility is currently under construction. LPA will return to facility to conduct a follow pre-licensing inspection to inspect physical plant once construction has been fully completed. (Continue Report on Page 2...) (Continued, Page 2...) Outdoors: Facility does not include an outdoor space. Applicant has applied for a waiver from department to utilize local park, "Mother's Meadow", located in Golden Gate park for outdoor space. Applicant has also applied to obtain a permit from city. LPA, applicant and director walked from facility to "Mother's Meadow" to inspect space. Park is approximately 0.3 miles from facility and is an approximate 10 minute walk from facility. LPA observed park to contain a play ground and open space field. LPA observed playground to have play structures with resilient padding underneath. Applicant and director are aware of teacher to child ratio when engaging in activities away from center, including utilizing local park, is 1 adult to 6 preschool children and 1 adult to 2 toddler option children. Applicant and director are aware water must be made accessible at park for children in care. Applicant and director are aware children are not to co-mingle with other individuals in the park or use the restroom in park when occupied by another individual. LPA reviewed with applicant and director the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Applicant and director were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Prior to recommended licensure, following must be complete: -Completion of construction in facility. -Fire clearance. -Approval of city permit to use local park. -Follow up pre-licensing inspection conducted by LPA.
