What the inspector wrote

On February 6, 2025, Licensing Program Analysts (LPAs) Van and Badger conducted an unannounced Plan of Correction (POC) inspection at the center, meeting with the Director, Wen Qi. The purpose of the inspection was explained, and access to the facility was granted. At the time of the inspection, the Director and a teacher supervised 11 children. During the annual inspection on January 9, 2025, several deficiencies were identified at the center. Notably, one individual present at the facility had not obtained a criminal record clearance. Additionally, the staff had not completed the required mandated reporter training, and cleaning products were found accessible to children. Following the January inspection, the center sent the staff's completed mandated reporter training certificate to the licensing office, along with a plan to ensure that all individuals working or volunteering at the center obtain criminal record clearances before being allowed on the premises. The director also mentioned that the adult without clearance was no longer associated with the center. During today\342\200\231s inspection, LPAs observed that all cleaning products were stored out of reach of the children and that all staff members present had criminal record clearances with eligible status documented on file. As of today, all deficiencies noted during the January 9, 2025, inspection have been resolved. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with the Director, Wen Qi.

This discrepancy was uncovered when the QR code on the certificate was scanned, which indicated that the certificate rightfully belonged to one of her teachers. This poses an immediate health, safety, or personal rights risk to persons in care.
