This requirement was not met as evidenced by:C1's diaper was not changed prior to leaving the center and as result C1 left the center with a soiled diaper. This is a potential risk to the health and safety of children in care.

This requirement was not met as evidenced by: LPA and Director toured the facility and observed unlocked cabinets with cleaning supplies in three out of five classrooms toured. This is a potential risk to the health an safety of children in care.

This requirement was not met as evidence by: Community Care Licensing was notified of an incident from facility staff and by unusual incident report that on 8/8/23, C1 was left unattended on the play yard of the facility for approximately three minutes. This poses an immediate risk to the health and safety of children in care.
