On 12/18/24, at 12:11 PM, following an unannounced random inspection, the LPA met with Site Supervisor, Misti Freeman, of the abovementioned CCC to discuss an incident that was reported to the department on 12/5/24. The following text details the conversation between the LPA and the Site Supervisor about an incident in which a child in care sustained a fractured arm. Child one, or C1, will hereby be referenced as the child who sustained the arm injury. Site Supervisor and one staff assistant, Cathy Shi, were outside when the incident took place with C1. Site Supervisor visually witnessed the incident. Site Supervisor was positioned near the play structure. Staff assistant was positioned closer to the modular classroom to view both the playground area and bike path area. Upon C1 suddenly jumping off the play structure, the Site Supervisor quickly accompanied C1 and noticed that the child broke their arm. Site Supervisor then walked the child to the nurse’s office and then contacted the parents right away. Approximately 10 minutes later, the parents came to pick up the child in the company of the Site Supervisor in the school's parking lot. Upon pick up, the Site Supervisor discussed what occurred to the parents. Because the child was crying, the Site Supervisor also used calming techniques to placate the child. Site Supervisor also reached out to Sierra Vista Hospital ER in San Luis Obispo, to let staff know what occurred and that Spanish was the family’s first language. At the ER, staff added a temporary cast to the child's arm. The child didn’t come to school the following day. The Site Supervisor attested that the child could’ve came to school, however, the mother wanted the child to rest and for her to look after him. C1 returned to school on 12/9/24. On 12/10/24, the child missed school for a follow up visit with an orthopedic surgeon in Santa Barbara where it was determined the child didn’t need surgery. A more permanent cast was added to the child’s arm at this time. The Director confirmed that the child is now okay and hasn't been dis enrolled from the program.
