What the inspector wrote

On November 13, 2025 Licensing Program Analyst (LPA) Elizabeth George conducted an unannounced Case Management Inspection. LPA met with Director Miranda Aguilar and discussed the purpose of the inspection to follow up on an incident reported to the Department on 11/3/25 as required. LPA in the company of the director toured the facility inside and outside. During the inspection, LPA observed 3 staff providing care to 12 children. On November 3, 2025 at approximately 2:30 PM, Staff 1 was lining up her class of 6 children in the hallway to go outside for outdoor play. During this transition child 1 was having a difficult time threw himself on the ground and refused to go outside with the class. The other children had moved through the door at this point to line up outside and staff 1 was on the outside of the door that leads outside. Staff 1 said to child 1 \342\200\230I guess you are staying with staff 2 and then proceeded to close the door rather forcefully. The child remained on the ground inside the door and immediately let out a scream and started crying. While staff 1 was attempting to get child 1 out the door staff 2 had walked into the bathroom to change a child, staff 2 immediately walked over to child 1 as soon as the door closed and the child started crying. Staff 3 & staff 4 were in the office during the incident and were able to hear the entire incident. Staff 3 immediately walked outside to check on the teacher and ask what happened. Child 1 then re joined his class outside for snack. Based on the information gathered during the inspection, LPA determined that the staff took appropriate action to meet the needs of child 1 and other children in care. No deficiencies were cited as a result of the incident. Exit interview conducted, a notice of site visit was given and report was reviewed with Director, Miranda Aguilar.
