What the inspector wrote

On 08/12/2025, at 10:45 AM , Licensing Program Analyst s (LPA s ) Seena Parsapour & Gigi Reyes conducted an unannounced Annual/Random inspection of the above-mentioned Family Child Care Home (FCCH). LPAs met with Mona Leyva, Licensee of the FCCH and explained the purpose of the inspection . LPAs in the company of the licensee toured the interior and exterior of the FCCH in its entirety. The FCCH uses the living room, dining room, one bathroom, and backyard for child care. The remainder of the FCCH is made inaccessible by use of child-safe door knob covers and child safe fencing. At the time of the inspection, LPAs observed seven (7) children under care and supervision. Upon inspection and after interviewing the licensee, LPAs observed a newly constructed room addition to the house, as well as a newly constructed accessory dwelling unit (ADU) on the property. Licensee admitted she failed to notify the Department of these renovations to the property. LPAs note that these additions to the property remain off-limits to children in care. LPAs also note that the ADU has a separate entrance from the FCCH, and a separate address. The FCCH is clean, orderly, and has ventilation to afford for the children\342\200\231s comfort. Sharps are stored in an elevated kitchen cabinet, and medications are stored in an elevated bathroom cabinet . Cleaning supplies are stored in the laundry room. LPAs note all locations which are inaccessible to children in care. The bathroom was observed to be clean and free of toxins. Toys, furniture and equipment observed in the FCCH are age appropriate. LPAs observed required licensing forms and documents posted on a bulletin board near the entry door of the FCCH . Continued on 809-C LPAs observed a dual smoke and carbon monoxide detector in the hallway of the home, which was tested at 11:36 AM and found to be operable. The FCCH has a regulation fire extinguisher (3A:40-B:C) that was serviced on 03/13/2025. LPAs reminded the Licensee to either service or purchase a regulation fire extinguisher annually. The home has a fireplace in the living room that is covered with mesh screening and blocked with furniture. The backyard is fully enclosed, and the entry/exit points are secured. The children have access to an area of the backyard that is separated from other areas of the backyard with child safety fencing. The backyard contains age-appropriate toys and play equipment. LPAs reminded Licensee to replace toys and play equipment when such items begin to degrade or are not in good repair. LPAs note there are no bodies of water on site. The children\342\200\231s records were reviewed. LPAs note that the children\342\200\231s immunization records are current and complete, but were not listed as required on the PM286 document. The children\342\200\231s records are otherwise current and complete with emergency contact information, and 15-minute sleep logs. The Licensee\342\200\231s records were reviewed and found to be current with CPR/First Aid certifications and Mandated Reporter training. LPAs note there is one dog in the FCCH. LPAs reviewed the dog\342\200\231s vaccination records. Licensee states there are no firearms or ammunition stored on site. Incidental Medical Services (IMS) policy was discussed. Continued on 809-C LPAs discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAs also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview the Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPAs completed the RSO profile in FAS. LPAs observed fire & disaster drill records, which were conducted and documented within the last 6 months; most recently, on 6/25/2025 at 11:00AM.     Based on observation and an interview with the licensee, the licensee did not comply with the section cited above in that CCLD was not notified of construction on the home premises, with respect to both the ADU and the room addition to the home, which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on record review, the licensee did not comply with the section cited above in that neither of the two assistants at the home have current mandated reporter training certification on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

  • Family Day Care Homes

    Based on record review, the licensee did not comply with the section cited above in that both of the assistants at the home do not have current immunization records on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
