What Happened
Annual inspection of CCC with infant and preschool components revealed one Type A violation (staff S4 not cleared through Guardian background check system creating immediate risk) and multiple Type B deficiencies including incomplete staff certifications, missing file documentation, and unfinished lead water testing.
Observations

- 15 children and 7 staff observed
- 2 classrooms total (infant and preschool)
- Preschool classroom: 1 sink, 1 toilet, changing station, potty
- Infant classroom: restroom with toilet, sink, changing station
- Staff restroom upstairs, off-limits to children, gated
- Classrooms clean and orderly
- Age-appropriate toys and equipment in good condition
- Cleaning supplies located upstairs, inaccessible to children
- Fire alarm, fire extinguisher, carbon monoxide detector observed
- CO detector tested 4:28 PM and operable
- Outdoor play areas fenced - preschool and infant yards separated
- Play structures and ample shade observed
- Water available via jugs and water bottles
- No firearms or ammunition stored on site
- No bodies of water
- TYPE A DEFICIENCY: Staff S4 not listed on Guardian roster - not cleared for criminal background as of 11/10/2025 (immediate risk)
- TYPE B DEFICIENCIES: 2 infant files missing updated infant needs/services plan (not quarterly updated); 2 staff files missing valid mandated reporter training, LIC503 Health History, and valid TB clearance
- LEAD TESTING NOT COMPLETED: Building water supplies not tested as of 11/10/2025
