What the inspector wrote

On November 2, 2023 @2:15 PM, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced case management visit in conjunction with an unannounced initial 10-day complaint inspection. LPA met with Director Michelle Lee and the purpose of the inspection was explained. Present in the facility today was Director, 12 staff (11 teachers & 1 teacher aides), 55 children. LPA inspected facility for Health and Safety Hazards. Case Management visit was for unusual incident report received on 09/07/2023. Unusual incident report stated a child had been hit on head with a brick by another child. Child who sustain injured required medical attention and cut was glued by medical doctor. Per lead teacher KIki, bricks and block and other building material were introduced in the classroom for a learning activity . Per Kiki, bricks were introduced to classroom to teach children how bricks and other materials can be used in a variety of ways. Facility uses real material for children to learn and get a sense of different types of material and their usage. Teacher stated a child was playing with the bricks to build and was holding a brick and turned around to place brick down when it accidentally slipped out of their hand and fell and hit a child on the head. Child who sustained injury was sitting on the flooring playing with other materials This was an isolated incident and an accident. Director discussed with staff the importance of extra supervision when learning materials used can cause injury to children, if not used properly. No deficiencies cited today under California Code of Regulations, Title 22, Division 12. Exit interview was conducted and report was reviewed with Director, Michelle Lee. A copy of this report was provided. Notice of site visit shall be posted and shall remain posted for 30 days.
