Supervision shall include visual observation.This requirement was not met, as evidenced by interviews, and documents collected. On 3/10/2022, C1 wandered away from care without staff knowledge. C1 was able to leave facility unattended through unlocked side gate.This poses an immediate health and safety risk to children in care. Although this is a ZERO TOLERANCE VIOLATION which results in A $500 civil penalty. No civil penalty will be assessed due to facility was self-reporting, no injury reported.
