What the inspector wrote

Due to facility being on winter break, LPA Tapia-Mandujano delivered the findings of the investigation to ECE Dean Tajmah Martin via phone on December 19th, 2023 at approximately 2:30pm. Complaint was received by the Department on 09/22/23. During the course of the investigation, health and safety inspections and staff interviews were conducted. LPA also received pertinent documents. Due to the interviews, inspections, and record reviews that were conducted, it was determined that there is not sufficient evidence to prove that a teacher hit a daycare child. Although the allegation above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the above allegation is UNSUBSTANTIATED. Upon receipt of this report, Facility shall post the Notice of Site Visit for 30 consecutive days. Failure to maintain posting as required will result in an immediate $100 civil penalty. This report is public and can be reviewed. After today’s phone call, an exit interview was conducted, report was emailed to ECE Dean Tajmah Martin at tajmah.martin@theprimaryschool.org. ECE Dean, Tajmah Martin has acknowledged receipt of report and a signed copy of the report will be received at a later date.
