What the inspector wrote

On February 20, 2026 at approximately 9:30AM, Licensing Program Analyst (LPA) Katie Krenn conducted an unannounced case management inspection. LPA met Assistant Director, Ilaise Meli (D2) and explained the purpose of today's visit. The director, Michelle Doan (D1) was not present during the visit. Present during the visit in addition to D2 were two staff supervising five infants. Adult to child ratio was met the facility was operating within it's capacity on this day. All staff present have completed criminal record clearances from the Department of Social Services and are associated with this facility on guardian. This infant facility operates on the same site as Whole Kid Preschool, facility number 414004816. Days and hours of operation are Monday - Friday from 8:00AM to 6:00PM. Per D2, there are no pools, ponds, or similar bodies of water on the property. LPA observed the required posting board and inquired if it was accessible to the public. Per D2, the postings board is not in an area that is accessible to the public. LPA interviewed D2 regarding pick ups and drop offs at the facility. LPA also observed the area where parents pick up and drop off children. LPA reminded D2 that parents need to be able to access the required postings. LPA and D2 discussed a few options for posting that would work for the facility and that would meet the requirements. LPA reviewed documents from staff files that were unavailable during the previous visit. At this time LPA is unable to verify that any of the staff currently caring for infants has completed three post-secondary semester or equivalent quarter units related to the care of infants, which poses an immediate health, safety or personal rights risk to persons in care. At this time LPA is unable to verify that one infant staff has been cleared for tuberculosis , which poses an immediate health, safety or personal rights risk to persons in care. continued on page 2 Page 2 Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Katie Krenn informed Assistant Director, Ilaise Meli that this report dated (February, 20 2026) documents two Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Katie Krenn informed Assistant Director, Ilaise Meli to provide a copy of this licensing report dated February, 20 2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/ guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Two Type A and four Type B Citations were issued during today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted, appeal rights and report were reviewed with the Assistant Director, Ilaise Meli.

  • PERSONNEL REQUIREMENTS

    (g) All personnel...(1)Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure. This requirement is not met as evidenced by:Based on file review, LPA determined that one staff is without verification of tuberculosis clearance, which poses an immediate health, safety or personal rights risk to persons in care.

  • ADMISSION PROCEDURES AND PARENTAL AND AUTHORIZED

    (c) The licensee shall post the PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster in a prominent, publicly accessible area in the child care center at all times.This requirement is not met as evidenced by: Based on interview with assistant director and observation of the area where parents pick up and drop off children. LPA determined that PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster is not posted in a prominent, publicly accessible location which poses a potential health, safety or personal rights risk to persons in care.

    This requirement is not met as evidenced by:(b) The center shall inform each child's authorized representative of the rights specified in (a) (1) through (8) above.(2) The center shall post a copy of the LIC 613A Personal Rights Form (9/96) in a prominent, publicly accessible area in the center. Based on interview with assistant director and observation of the area where parents pick up and drop off children. LPA determined that LIC 613A Personal Rights is not posted in a prominent, publicly accessible location which poses a potential health, safety or personal rights risk to persons in care.

    (f) The licensee shall post signs at the entrance to the child care center that provide the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5 This requirement is not met as evidenced by:Based on interview with assistant director and observation of the area where parents pick up and drop off children. LPA determined that the telephone number of the local health department and information on child passenger restraint systems are not posted in an area regularly accessible to parents and caregivers which poses a potential health, safety or personal rights risk to persons in care.

  • INFANT CARE TEACHER QUALIFICATIONS AND DUTIES

    (b) Prior to employment, an infant care teacher shall have completed, with passing grades, at least three postsecondary semesters or equivalent quarter units in early childhood education or child development, and three postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university. This requirement is not met as evidenced by:Based on file review, LPA determined that none of the staff currently caring for infants has completed three postsecondary semester or equivalent quarter units related to the care of infants, which poses an immediate health, safety or personal rights risk to persons in care.
