What the inspector wrote

On July 23, 2024, Licensing Program Analyst (LPA) Susana Martinez conducted an unannounced Required-3 Year inspection. LPA met with director Lola Gonzalez and advised her the purpose of the inspection. Director provided LPA a tour of the facility inside and out. This is a combination Preschool and School age program located on the grounds of Bethania Lutheran Church. There were 30 children in care at the time of the inspection along with 4 staff. Center operates M-F 7:30 AM - 5 PM. LPA observed required licensing documents mounted on the wall at the entrance of the facility. Currently the facility uses a paper sing-in/sign-out sheet. LPA verified that the number of children signed in for today correlate with the number of children present on site. Last fire drill was observed to be conducted on 03/12/2024. Daily activity schedules were observed thought the facility. Lunches are brought from home, the center provides snacks. LPA observed snack menus posted, Director states snack menus are posted online for parents to view. Children's Roster was observed to be complete. The center currently uses four rooms for care. Classrooms are equipped with age-appropriate toys, teaching materials, and furnishings for children in care. LPA observed enough restrooms available for children to use. Bathrooms are clean and sanitary. LPA did not observe any hazards/toxins items accessible to children. First Aid kits are located throughout the facility. Sleeping cots were observed to be readily available inside the classrooms. LPA observed dual smoke/carbon monoxide detectors throughout the facility, director states detectors were checked recently to verify they are functioning. There is drinking water readily available for children inside and outside the classrooms. The outdoor playground is fenced with appropriate shading and has an ample amount of space for children to play. Waste containers have tight-fitting covers. LPA observed the playground has age-appropriate toys and structures available for children to use. The areas around or under high climbing equipment have cushioned material. No bodies of water were observed on the premises. Continued on 809-C A sampling of children and staff records were reviewed. LPA reviewed a random sampling of 5 children's files. Files were observed to be complete and current. LPA reviewed 5 staff files. LPA observed 3 out of 5 staff mandated reporter certifications to be expired. LPA verified SB792 Child Care Adult Immunization and Tuberculosis requirements. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Center re-tested in December 2022 after conducting remedial plan. LPA verified new results lead exceedence is less than 5 ppb after remedial plan. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Continued on 809-C Site Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. One Type B deficiency for missing mandated reporter certifications was issued during today's inspection and can be found on the attached 809-D. One Technical Advisory was issued as a reminder to update the physician's report to reflect the child's current medical needs. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted, appeal rights were provided and report was reviewed with the Director, Lola Gonzalez.
