What the inspector wrote

On 4/10/2026 Licensing Program Analyst (LPA) German Negrete made an unannounced inspection for the purpose of conducting a Case Management – incident inspection. LPA met with Lead Teacher Gloria Ochoa and discussed the purpose of the inspection. During today’s inspection, LPA did a walk through of the Child Care Center, inside and out. LPA notes at the time of the inspection there were 10 toddlers receiving care and supervision from 5 staff members. On 4/03/2026, Facility Representative contacted Community Care Licensing (CCL) to self-report the following unusual incident: Child#1(C1) had a seizure at approximately 8:50am after breakfast. C1 was sitting down next to classroom table and was observing the other children, staff (S1)noticed the child was shaking and eyes were blinking. Then, S1, observed C1 collapse and C1's laid flat on the ground. C1 was not responsive. S1 called S2 and S2 immediately went over to observed the child and S2 informed staff they need to contact 911. CCC staff contacted 911 and Authorized representative of C1. Then C1 was transported to Lompoc Valley Medical Center. Today LPA verified the LIC624 was submitted to CCL in the appropriate time frame as outlined in Title 22 California Code Regulations. Continued on LIC809-C(pg2) Today LPA conducted interviews, including with Child #1’s (C1) authorized representative (P1). P1 stated the Child Care Center (CCC) promptly notified them within minutes when C1 appeared to require medical attention. This marks the end of CCL's review of the incident. The Department reserves the right to reopen the review if additional information is received. The CCC will not be receiving any citations today. Appeal Rights were provided. A copy of this report was emailed to Facility Representative Shonna Martin. Notice of sight visit was provided via email. Notice of site visit was provided as required by Health and Safety Code Section 1596.817 and must remain posted in a prominent publicly accessible area in the center for 30 days, or a civil penalty of $100.00 will apply.
