On April 23, 2025 at 9:00 AM, Licensing Program Analyst (LPA) Gigi Reyes conducted an unannounced Annual Inspection at the above Family Childcare Home (FCCH). LPA met with Licensee, Dora Alapizco and discussed the purpose of the inspection. FCCH operates Monday to Friday, 4:00 AM to 11:30 PM, Saturday 4:00 AM to 3:00 PM Licensee provides care for children aged 0 months to 13 years old. Upon arrival, LPA Reyes observed a total of 16 children in the FCCH - 3 infants, 8 school age children, and 5 toddlers under the care of 3 staff members including licensee. LPA and Licensee toured the home. LPA observed that required forms are posted in prominent location. Smoke and carbon monoxide detectors were tested and found functional. Fire Extinguisher was serviced on 4/17/2025 Age-appropriate toys, books, playpen, and equipment were observed in the home. Children’s bathroom is free of toxins . Sharps, medicines, and toxins are stored in the area inaccessible to children in care. The backyard is enclosed with wooden fence. LPA reviewed the facility file and confirms that Licensee has a current Pediatric CPR and First Aid certificate which expires on 8/20/2025 while Mandated Reporter Training expires on 2/5/2027. Children’s records were reviewed, Licensee and staff members check and document each napping infant every 15 minutes. Upon review of Facility and children's files the following deficiencies were observed: Staff 2 does not have immunization and tuberculosis test result on file Staff 3 does not have record of immunization on file Adult 1 who lives in the home does not have tuberculosis test result Infants # 1, 2, and 3 do not have LIC 9227 - Individual Safe Sleep Plan on file Infants # 1, 2, 3, Child # 4,5,10,15,16 their immunization are not documented on PM286 or CDPH 286 Infant # 3 does not have immunization on file During the record review, it was observed that most of the children's forms were incomplete, dates are left blank and unsigned. LPA provided licensee the list of records that must be maintained in a FCCH. LPA also explained each form to the licensee. LPA did not observe bodies of water. Licensee stated that no guns and ammunition are stored separately in a safe in the inaccessible area. Licensee, was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee, Dora Alapizco the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The FCCH is not providing Incidental Medical Services (IMS). IMS policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. During the exit interview, the LICENSEE, Dora Alapizco confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. During today's inspection, Type A and Type B deficiencies were cited under Title 22 Division 12. and Health and Safety Code. Technical Violations were issued. LPA Reyes informed licensee, Ms. Alapizco that this report dated 4/23/2025 documents one (1) Type A citation which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care. Also, LPA Reyes informed the licensee, Ms. Alapizco to provide a copy of this licensing report dated 4/23/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. .
