The report is amended on 8/12/2025 by LPA. On 7/22/25, Licensing Program Analyst (LPA) Joaquin Mendez conducted an unannounced Case Management Inspection of the abovementioned Family Child Care Home (FCCH) to address an incident report received by CCLD on 7/13/25. The provider provided LPA with an account of what transpired and re-enacted the incident. Specifically, C1 was on a walk-through Liberty Park with the licensee when two dogs showed up. C1 tried to touch one of the dogs and a dog bit C1 on the upper part of the left shoulder. The provider is unsure which of the dogs that bit C1 was. C1’s parents were called to inform them of the incident. Parents arrived and took C1 home. The provider states she asked parents if they wanted her to take C1 to the doctor, but parents refused. The provider states parents stated C1 tries to touch strange dogs all the time. LPA asked the provider if C1 is on site. The provider affirmed C1 is not present today. C1 returned to FCCH on 7/14/2025 and parents stated to the provider that C1 is fine. The licensee provided the current license and immunization for her personal dogs. One dog (Chico) was on the walk with the licensee and C1 during the incident. · Chico-A437983 license expires 2/27/2026 · Martina- A437984 License expires2/27/2026 No deficiencies were cited during today's inspection. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements may result in a civil penalty of $100. Exit interview conducted and report was reviewed with Licensee Dora Alapizco in preferred language, Spanish.
