What the inspector wrote

On 03/27/2026, at 02:03PM, Licensing Program Analyst (LPA) Fernando Hernandez conducted an unannounced Case Management - Incident Inspection at the above-mentioned Family Child Care Home (FCCH). LPA met with Licensee Denise Alonso. LPA discussed the nature of the inspection with the licensee. During the inspection LPA observed (5) children present under the licensees care and supervision at the FCCH. The Licensee self reported on 03/12/2026 an incident that occurred on 03/11/2026 that a child's (C1's) parent left a voicemail stating C1 has anemia and "may not make it". According to the licensee parent and licensee have discussed since September C1 has not been wanting to eat. Parent asked the licensee if they had noticed C1 breathing strangely, licensee stated they did not notice any concerns with the child, licensee stated they advised the parent to seek immediate medical attention. Licensee stated parent took C1 to the emergency room and proceeded to contact licensee via telephone call requesting if C1 has eaten. Licensee informed parent C1 did not want to eat and would not force C1 to eat. Licensee stated the parent informed the licensee that "it would be the licensees fault if something were to happen to C1 and someone would be contacting you soon". Licensee stated she informed the parent they would be willing to speak with anyone regarding the incident. According to the Licensee, both the parent and licensee mutually agreed to discontinue child care service on 03/11/2026. Interview with the Licensee informed LPA an inspection with Early Development Services occurred on 03/18/2026 which they informed Licensee the C1s parent spoke with them and informed the program C1 was doing fine. LPA reminded Licensee that it is their responsibility to report any unusual/incidents to the department.. During today’s inspection no deficiencies were issued. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with Licensee, Denise Alonso.
