What Happened
Case management follow-up on self-reported incident from 06/24/2025 where a volunteer parent provided an allergen-containing item to a child. Staff responded appropriately by removing the item and contacting parents. Type B citation issued.
Observations
- Volunteer parent provided child C1 with muffin containing documented allergen without staff knowledge or permission
- Child had access to small bite of muffin
- Staff discovered item, swiftly removed it, and called Director
- No adverse reaction observed during time in care
- Parents verified no adverse reaction next day; child cleared by emergency clinic physician
