What the inspector wrote

Based on the LPA Record Review, Unusual Incident Report (UIR) LIC 624 dated 5/17/2024, showed that on 5/9/2024 at 10am, C1 left unattended in a small room outside of C1's classroom corridor in between classroom and outdoor space for 2-3 minutes. The UIR copy was emailed to the Regional Office email on 5/19/2024 at 3:04 pm. Based on LPA's Staff interview, on 5/9/2024 at 10am, S2 found C1 on top of a play structure, unsupervised, in the corridor right across of S2's room. S1 confirmed that C1 was left unattended for about 2-3 minutes. S2 informed S1 about the incident. The Executive Director was only informed a week later regarding the incident. The preponderance of evidence standard has been met, therefore the above allegation is to be SUBSTANTIATED. The Licensee were in violation of Section 101229 (a)(1) that no child shall be left without the supervision, including visual supervision, of a teacher at any time. LPA Estoesta informed the Executive Director that this report dated 05/22/2024 included a Type A Citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA's informed Executive Director to provide a copy of this licensing report dated 5/22/2024 that documents of any Type A citation, to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
