What the inspector wrote

On 07/30/2025 at 9:35am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu met with Director Mayra Howard for an unannounced case management inspection . Present during the inspection were director, 13 fingerprint-cleared staff, and 68 children in care. The facility is within ratio and in compliance with capacity regulations today. Facility is located on the premises of Cubberley Community Center and was toured to conduct a Health and Safety Inspection. The facility shares a campus with an infant license (facility #: 434400297). The facility has separate indoor and outdoor spaces for infants and preschool children to ensure there are no comingling. During the inspection, the ground surrounding the play structure in the 2's yard is padded with black rubber mats. However, LPA observed multiple rubber mats are lifted in the corners, which is a trip hazard to the children. Per Director and LPA's previous observations at the facility, there used to be black rubber mulch on top of the mats. However, the rubber mulch has been removed as parents and teachers reported the mulch leaving black residue on the children. LPA verified that the director is aware of the situation and is actively working to replace the mats pending required permits from the city. The current plan is to remove the mats and use wood chips as padding. LPA also observed a large umbrella used for shade in the 2's yard had a loose rod hanging down. Children and teacher were under the umbrella. However, the rod is at a height that does not affect the children, and LPA determined it is not currently a health and safety risk to children in care. LPA advised Director this umbrella shall be removed immediately as the integrity of the umbrella may be affected. Director stated they have backup umbrellas that can be used for shade. No deficiencies were cited during today's inspection. However, LPA advised Director the issues addressed above shall be resolved as soon as possible or it may result in future citation(s). A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Mayra Howard.
