What the inspector wrote

On 05/06/2026 at 8:30am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an unannounced case management inspection for the purpose of investigating two (2) reported incidents. LPA met with Director Mayra Howard. Also present during the inspection were five (5) fingerprint-cleared staff and 42 children in care. The facility is within ratio and in compliance with capacity regulations today. One incident occurred on 02/26/2026 and was self-reported to Licensing on 03/06/2026. Another incident occurred on 03/06/2026 and was self-reported to Licensing on 03/06/2026. Both incidents were regarding interactions between a staff and a child (different in each incident). After the incident was reported to the Director by witnesses, Director conducted internal investigation of the incident. During today’s inspection, LPA reviewed the above information with Director and conducted interview to obtain more information. The two (2) staff involved in the incidents are no longer employed at the facility as of today’s inspection. Following the incidents, Director reviewed Personal Right’s regulations and expectations with staff in team meetings. LPA obtained agenda for the team meetings. Based on information available, LPA determined the above two (2) reported incidents involved actions towards children that were punitive in nature. One (1) Type B deficiency regarding Personal Rights was issued during today’s inspection. The citation was cleared at the end of today’s inspection as LPA determined Director has already taken appropriate actions to address the deficiency. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Mayra Howard. Page 1 of 1.
