What the inspector wrote

On 12/8/2021 approximately 8:45am Licensing Program Analyst (LPA) Kelly Phan conducted an UNANNOUNCED ANNUAL REQUIRED INSPECTION . LPA Phan met with Sepideh Sotoudeh Facility Director, also present at the time of this inspection were 13 staff members and 57 preschool aged children. The facility is within ratio and capacity compliance today. The facility was toured to conduct a Health and Safety Inspection. This program current operates Monday-Friday 8:30am-5:00pm out of preschool areas 1-8 and library. The isolation areas are the library and staff room when applicable. The classrooms (butterfly, dolphin, duckling, Jr-Pre-K, and Pre-K) were inspected for age appropriate furnishings, equipment, and adequate storage for children’s belongings. LPA observed the cleanliness of floors and surfaces, the presence of working carbon monoxide detector, smoke detectors/fire alarms and a fully charged fire extinguishers size 2A10BC that is accessible throughout the facility. The outdoor play area is fully fenced. There is padding under/around the climbing equipment. Drinking water is accessible inside and outside of the facility. LPA observed the play structures to be fully intact with no visual defects or concerns. There are no pools, hot tubs or other accessible bodies of water. Per director, there are no firearms present on the premises; Parents also provide snacks/meals for the children due to COVID. Cleaning supplies/toxins are stored inaccessible to children during the inspection and all required postings are present along with children's allergies listed in each classroom. LPA reviewed storage of food and medications and were found to be in original packaging and labeled with expiration dates along with child's names. The center also conducts and documents fire and earthquake drills once every 6 months and the logs show that drills were conducted on 9/23/2021. The sign in/out binder was reviewed and has 41 signatures present, as there were some children who only comes for half a day. LPA also talked to director about wearing masks for children 2 years and older indoors and to refer to PIN and CDSS website for reference Please see LIC 809-C for continuance At 11:45AM, 12 children files and 8 staff files were reviewed and most were found to be complete; LPA informed that there were some files that were missing/incomplete (LIC 627 & LIC 503) and were able to correct them today. A copy of the facility personnel roster and children’s roster were available for review and copy was obtained. LPA reviewed that the staff files had fingerprint cleared, completion of Mandated Reporter training as well as proof of immunizations. All staff have current CPR/First Aid certificates, and proof of education and/or experience was also present. The first aid backpack was observed and found to be complete. LPA also discussed the usual injury/incident report to be called into the office within 24hrs and fax a copy of the report within 7 business days. Exit interview conducted. Appeal rights and notice of site visit was given to director to post for 30 days. There were no deficiencies cited as of today's inspection. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
