What the inspector wrote

On 10/24/2025 at 8:30am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Case Management - Incident visit and met with Director (Saman Tafazoli) and owners/licensees (Simin Parvini & Cyrus Parvini). Also present at the time of today's visit are 13 staff and 69 children. The purpose of today's visit is to discuss and follow up on two unrelated incident reports submitted to the Department (CCLD) by facility representatives recently on 09/25/2025. One report dated 09/25/25 claims that a child injury had occurred on 09/19/25 and the facility failed to follow reporting requirements to the Department. A second report dated 09/25/25 details an incident which occurred at the facility involving a former staff member. LPA Uribe interviewed several staff members including the director, owners/licensees, and administration staff regarding the incidents. Copies of relevant documents were also obtained. LPA Uribe offered consultation to the facility representatives and held further discussions. It is determined that the claim that the facility failed to report a student injury is unfounded as the owners/licensees provided documented proof that no such violation occurred. The second and unrelated incident that occurred on 09/25/25 involved a former staff member whose employment was terminated and was removed from the premises. It is determined that no children/clients were involved or a witness to the incident that occurred. The incident itself did not disrupt or interfere with the operations of the facility. No deficiencies were cited during today's visit. Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the director, Saman Tafazoli.
