Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required 1- Year inspection. LPA met with Director Jennifer Kwong and Assistant Director Michael Frugoli and explained the reason for the inspection. LPA entered the facility at 1:52PM. Licensee Kamal Gill arrived shortly after. There is an area to post required postings, such as license, car seat, notification of parent's rights, and personal rights. The hours of operation of Monday through Friday 6AM to 6:30PM. Facility uses an electronic sign in/out. LPA reviewed the electronic sign in/out. There were 81 children signed in today. LPA toured the inside and outside of the center. LPA observed that there were bottles of disinfectant spray in the cabinet in Preschool Classroom #2, next to the utility room, and disinfectant spray on the pony wall in Preschool Room #2 that is in-between the Toddler Room and Pre-K room. Licensee Kamal moved the disinfectant spray to an area that is inaccessible during today's inspection. LPA also discussed with Licensee and Assistant Director Michael that diaper creams and sunscreen to be inaccessible to children. LPA observed that there a container of medication in a staff's bag, which was accessible to child. Staff moved bag during today's inspection. The bathroom for children were observed to in functioning condition. There is a fully charged fire extinguisher, smoke detector, and carbon monoxide detector. The last disaster drill was conducted on 04/27/2023. Licensee stated that there are no weapons, such as firearms, stored on the premise. -----------------------continuation of 809 dated 06/14/2023 page 2----------------- --------------------continuation of 809 dated 06/14/2023 page 1--------------------- The outdoor area is fenced. Shaded rest area is provided through trees, building overhang, and canopy. Area around play structure has resilient material, but does not have enough to absorb fall. LPA discussed with Licensees to check all equipment for any crack. LPA observed that one area of the grass had water and was muddy causing it to be slippy. Licensee placed an empty wading pool to cover the area. There were no bodies of water observed during today's inspection. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. C-1's medication expired on 03/2023. Licensee followed up with parent during today's inspection to obtain an updated medication. For IMS information, see PIN 22-02-CCP. The records reviewed include but not limited to immunization record and parent's rights. Licensee Kamal was informed that informed that the Annual Inspection will continue and be completed at a later date. As a result of this inspection, a Type A and Type B citations were issued. Exit interview conducted and report was reviewed with Licensee Kamal Gill. A notice of site visit has been issued and must remain posted for 30 days. LPA Samantha Yip informed Licensee Kamal Gill that this report dated 06/14/2023 document(s) one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Samantha informed the facility representative to provide a copy of this licensing report dated 06/14/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
