101227(a)(1) Food Service: (a) In childcare centers providing meals to children, the following shall apply: (1) All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children...This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above by not following the centers food menu that was submitted to CCL and to parents via email, which poses a potential health, safety or personal rights risk to persons in care.

FOOD SERVICE

Food Services. Menus shall be in writing and shall be posted at least one week in advance in an area accessible for review by the child's authorized representative.This requirement is not met as evidenced by: Based on interview and record review, facility did not offer the morning snack that was listed on the menu, which poses a potential health and safety risk to children.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time,... Supervision shall include visual observation.This requirement is not met evidenced by: Based on observation, LPA observed at 2:50PM that a child walked from the bathroom to the classroom without any staff present and at 2:54PM, a child walked from the classroom to the cubbies without any staff present. This poses an immediate risk to the health and safety to the children in care.

TEACHER-CHILD RATIO

Teacher-Child Ratio. There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.This requirement is not met as evidenced by: Based on record review and observation, Pre-K had 25 children with 2 fully qualified teachers, Preschool #2 had 24 children with two staff, whom one was only fully qualified, and preschool room #5 had 18 children with 2 staff, whom none are fully qualified teacher. This poses a immediate health and safety risk to children in care.

TEACHER QUALIFICATIONS AND DUTIES

Teacher Qualifications and Duties. To be a fully qualified teacher, a teacher shall have one of the following: Twelve post-secondary semester or equivalent quarter units in early childhood education or child development completed, with passing grades, at an accredited or approved college or university; and at least six months of work experience in a licensed child care center or comparable group child care program.This requirement is not met as evidenced by: Based on record reviews, the middle area did not have a fully qualified teacher in the room and in Preschool #2, there was 24 children with only one fully qualified teacher. This poses an immediate health and safety risk to children in care.

Based on observation, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. S-2 was supervising 24 children with a fully qualified teacher. S-2 has only completed 8 semester units and is not enrolled in additional courses.

CRIMINAL RECORD CLEARANCE

Based on record review, the licensee did not comply with the section cited above, which posed a potential health, safety or personal rights risk to persons in care. S-1's eligible fingerprint was disassociated to facility. S-1 is associated to Licensee's other facility.

TEACHER-CHILD RATIO

Based on observation, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. LPA observed at 232PM that children were starting to wake up from nap. There were 28 children and two staff.

BUILDINGS AND GROUNDS

Based on observation, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. LPA observed that there was a spray with disinfectant spray on the pony wall and medication in a bag that was accessible to children.

OUTDOOR ACTIVITY SPACE

Based on observation, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. LPA observed that there is not enough resilient material to absorb falls.

Based on record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. C-1's medication expired on 03/2023.

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f)...This requirement is not met as evident by: Based on record reviews, S-1 has fingerprint clearance and is associated to infant license, which poses a potential health and safety risk to children in care.
