Routine inspection (3-year)·January 9, 2025

Licensing Program Analysts (LPAs) Mel Matos and Syeda Bahar met with Lucy Nunez, Director, for an unannounced Required - 3 Year inspection. Lucy states that she recently started at the Facility on Monday January 7, 2025 and is in process of submitting her director paperwork to the Department. Director states that the Facility has been closed since 2022 and is in process of setting up to reopen in the near future. Director states that she is not sure of the "reopening" date at this time. LPAs provided Director with the required documents for Director and Licensee Representative along with Records To Be Maintained at the Facility (LIC 310A) and Entrance Checklist - Child Care Centers (LIC 125). Exit interview conducted and report was reviewed with the Director, Lucy Nunez. No deficiencies issued during today's inspection. A Notice of Site Visit was given and must remain posted for 30 days.
