What the inspector wrote

Based on investigation, file review, and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations (Title 22, Division 6, Chapter 8), is being cited on the attached LIC 9099D. A notice of site visit will be issued and must be posted for 30-days.

What does Type A vs Type B mean?

Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.

Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

    (1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights...(2) Upon enrollment of a new child in a facility, the licensee shall provide to the parents or legal guardians of the newly enrolling child copies of any licensing report that the licensee has received during the prior 12-month period that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care...This requirement was not met as evidence by: Based on observation and record review the licensee did not comply with the section cited above by not having all parents sign the LIC 9224 when the Type A citation happened (OCT 2023) and when they have newly enrolled students into this facility which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review the licensee did not comply with the section cited above by not having all parents sign the LIC 9224 when the Type A citation happened (OCT 2023) and when they have newly enrolled students into this facility which poses a potential health, safety or personal rights risk to persons in care.
