What the inspector wrote

Licensing Program Analyst (LPA) Jennifer "Jen" Beehler conducted an unannounced Case Management - Other Inspection. LPA was provided access to the facility by the Director. LPA stated the reason for the visit. LPA reviewed the Guardian Roster for the facility and observed Staff Person #1 (S1) was not associated on Guardian Roster and a Criminal Record Clearance could not be located from the name provided. Licensee conducted a search based on the Personal Identification number that was listed on the Live Scan documents. This number revealed the staff person did have Criminal Record Clearance, but was not associated on the Facility Roster. Licensee associated S1 to the facility roster. Due to today's inspection, a deficiency is cited. Exit interview conducted with Director Danna Osborn. Report was reviewed and provided along with appeal rights. NOTICE OF SITE VISIT PROVIDED AND MUST REMAIN POSTED FOR 30 DAYS.
