What the inspector wrote

Licensing Program Analyst (LPA), Jennifer \342\200\234Jen\342\200\235 Beehler, met with Licensee, Radha Gopal Krishna , for an unannounced Required- 3 Year Inspection. LPA was granted access to the home by the Licensee and toured both indoors and outdoors during the inspection. Upon arrival, there were 12 children (school-age) and 1 staff (licensee) present, which is not compliant with the home license capacity and ratio requirements. LPA observed all required postings near the entrance to the home. Hours of operation for the facility are Monday \342\200\223 Friday, 03:00 PM-06:00PM and 8:00 AM - 6:00 PM on days when the local school has a holiday. Licensee states that adult s, over the age of 18, residing in the home are: herself and her spouse (Venkata Ramanan) . Licensee, Radha Gopal Krishna was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. All adults have criminal record clearance and are associated on the Guardian Roster for the facility. LPA reviewed facility roster (LIC9040) and fire/disaster drill log during todays inspection. The last fire/disaster drill was conducted on 12/15/2025, which is compliant with the six month requirement for homes. LPA observed a fully charged 2A10BC fire extinguisher manufactured in 2019 which is not compliant with State Fire Marshall Regulations. Continued on Page 2 LPA observed a fully functioning smoke detector and carbon monoxide detector. Licensee states that she does not currently have any children in care who require Incidental Medical Services and does not administer medication at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Indoor areas of the home were inspected by the LPA today and observed to be clean, orderly, and safe for the day care children. Off-limits areas of the home (indoors) include all living space with the exception of the family room and hall bathroom. There are no open-faced heaters or fireplaces in the home. LPA observed sufficient age-appropriate materials and activities in the home. Furniture, such as tables, chairs, and shelves, are in good condition and safe for children. Drinking water is readily available for children in the facility via water pitcher with cups available if children forget their reusable water bottles from home that are labeled with children's names. The bathroom used for the day care is clean, sanitary, and operable. The Licensee has a working telephone in the facility. The backyard area of the home was inspected. LPA observed seating area, but no toys for children. Licensee stated the children do not use the space except to exit the home during the fire drill. Off-limit areas outside include side yards and front yard. No bodies of water were observed during today's inspection. LPA observed the licensee with the children. The class was split into three groups and a trivia game was played. Children were observed completing homework, reading or art activities. Six (6) children\342\200\231s files were reviewed during todays inspection for the following documents: Notification of Parents Rights (LIC995A), Consent for Emergency Medical Treatment (LIC627), Identification and Emergency Information (LIC700), Affidavit Regarding Liability Insurance (LIC282), Additional Children in Care (LIC9150). Continued on Page 3 1 staff file (Licensee) was reviewed for Mandated Reporter Training Certificate, and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza or statement declining influenza. Licensee has current CPR/First-Aid that expires 07/25/2027. The Licensee has current Mandated Reporter Training which expires 10/17/2026. LPA reminded Licensee that training must be renewed by all staff every 2 years. Supervision of children was discussed with the Licensee and she understands that she must be home during day care hours and ensure that children are supervised at all times. The Licensee states that she does not transport any day care children. LPA reminded Licensee that children should not be left unattended in parked vehicles and that car seats shall only be used for transportation and shall not be used for sleeping. LPA discussed ratio requirements with the licensee and reminded her that if she does not have an assistant present, she cannot have more than eight (8) school age children in care. During today's visit the following documents were reviewed and/or collected: LIC279 - an updated copy was collected LIC610 - document was reviewed LIC9150 - a blank copy was provided to the Licensee As a result of todays inspection, one (1) Technical Violation, one (1) Type A Citation and one (1) Type B Citation were cited. During the exit interview, the Licensee, Radha Gopal Krishna, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Exit interview conducted with Licensee, Radha Gopal Krishna. Report was reviewed and provided along with appeal rights. Continued on Page 4 LPA informed licensee Radha Gopal Krishna that this report dated 01/14/2026 documents one (1) Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed the licensee to provide a copy of this licensing report dated 01/14/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPA provided Licensee with a LIC9224 with today's date. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
