What the inspector wrote

Licensing Program Analysts (LPAs) Mel Matos & Fermin Campos-Jaramillo, conducted an announced prelicensing inspection at the Facility. LPAs met with Li Mock, Applicant Representative, and Chinlan Wang, Director, and explained the nature of today's inspection to them. The Applicant has applied for a license for 80 preschool children (including 28 toddler children) at 1321 Miller Ave, San Jose, CA 95129. No fire clearance approval has been received from the San Jose Fire Department. The Facility will operate Monday through Friday from 7:30 AM to 6:00 PM, serving Preschool children ages 2 years to 6 years and Toddler option children ages 18 months to 36 months of age. The Preschool will be utilizing three classrooms and the Toddler option will utilizing one classroom. Both Preschool and Toddler option programs have separate indoor and outdoor space, including bathrooms/sinks. Updated Facility sketches were provided to LPAs during today's inspection LPAs toured and measured the indoor and outdoor areas of the Facility with Li and Chinlan during today's inspection. INDOOR MEASUREMENTS ARE AS FOLLOWS: Classroom 1: 21.333 x 41.500 = 885.319 minus 11.281 encumbered space = 874.038 Classroom 2: 41.583 x 21.333 = 887.090 minus 50.677 encumbered space = 836.413 Classroom 3 (Toddler option): 43.417 x 23.417 = 1,016.695 minus 36.877 encumbered space = 979.818 Classroom 4: 12.667 x 9.583 = 121.387 Preschool Indoor Space Total = 2,811.656 sq. ft. divided 35 sq. ft. = 80 preschool children. There are five sinks (75) and five toilets (75) available for the preschool children. There are three (45) sinks and two (30) toilets available for the toddler option children. There is no hot water available in the children's sinks. LPAs observed soap dispensers and paper towels in the preschool and toddler option bathrooms. There is a changing table in the toddler option bathroom that is age appropriate and within arm's length of a sink. There is one separate staff bathroom, located near the front of the Facility, that will be utilized by sick children if necessary. Sick children will be isolated in the front office and isolation equipment will be provided. Potty training will be provided by the Facility. LPAs observed no fireplaces or wall heater units in the Facility. All rooms have appropriate lighting, heating, and air conditioning. First aid kits are stored in each Classroom inaccessible to the children. The Facility has working smoke/carbon monoxide detectors and a centralized fire alarm system. There are 10 tables, 63 chairs, 52 cubbies, and 42 cots available for the preschool children. There are 4 tables, 31 chairs, 28 cubbies, and 32 cots available for the toddler children. There is adequate equipment, supplies and toys for the children in the Facility. Drinking water will be provided indoors via reusable water bottles and filtered tap water. LPAs advised required water lead testing within 180 days of receiving license for Child Care Centers. There is a working telephone at the site. The phone number is (408) 473-3574. Li states that there are no firearms or weapons on the premises. The food preparation area (kitchen) is functional and has hot and cold running water, sink, refrigerator, microwave, dishwasher, and storage space for food supplies. The Facility will only provide AM/PM snack to the children. The children will be required to provide their own lunches or order from a third party catering service that will deliver meals to the Facility daily. Cleaning and sanitation of dishes and utensils will be done via dishwasher. The Facility is planning to primarily utilize disposable products such as utensils and plates. LPAs observed solid waste containers with tight-fitting lids throughout the Facility. Cleaning supplies are stored in the upper cabinets of the kitchen area and each Room inaccessible to children. There are no poisons in the Facility. Li Mock, Applicant Representative, and Chinlan Wang, Director, have obtained the Criminal Record and Child Abuse Index clearances. Chinlan has the required education and experience qualification requirements and valid Mandated Reporter Training & Pediatric First Aid/CPR Certifications. Proof of completion of Preventative Health Practices and vaccination records for MMR, Tdap, and Influenza are on file. OUTDOOR MEASUREMENTS ARE AS FOLLOWS: Playground A (Toddler option): 61.417 x 27.583 = 1,694.065 minus 135.539 encumbered space = 1,558.526 Playground B: 73.417 x 34.417 = 2,526.792 Garden: 86.333 x 16.167 = 1,395.745 minus 9.480 encumbered space = 1,386.265 Playground C (Toddler option): 35.667 x 32.000 = 1,141.344 TOTAL OUTDOOR SPACE: 6,612.927 sq. ft. divided by 75 = 88 children. LPAs observed that the preschool and toddler option playground areas have sufficient space for the preschool & toddler option children and are separate/surrounded by appropriate fencing. However, the bottom of three fence sections of the Garden area need to be covered since the gap measures LPAs observed sufficient outdoor supplies and equipment for the children. LPAs observed a play structure in Playground 1 and Playground 2 that is age appropriate & secure with rubber surfacing (resilient materials) underneath and surrounding the structures. Shade is provided by building overhangs, sun sails, canopies, and trees. Drinking water will be provided via children's reusable water bottles that will be refilled by staff prior to going outdoors. LPAs did not observe any bodies of water during today's inspection. Transportation will not be provided by the Facility. This Facility does not plan to provide Incidental Medical Services – IMS at this time. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. LPAs advised Li and Chinlan that an IMS plan will need to be submitted to the Department if they elect to provide IMS services in the future. Copy of PIN 22-02-CCP provided to Li and Chinlan during today's inspection. LPAs reminded Li and Chinlan of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the facility, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day up to $500.00 per person. For a subsequent violation within a 12 month period, civil penalty amounts to $100.00 per person up to $3000.00 per person. LPAs reviewed with Applicant, Li Mock, the LIC 311A, Records to be Maintained at the Facility (children’s forms/records, facility forms/records, and information to be posted).
