What the inspector wrote

An annual inspection was conducted at the facility by Licensing Program Analyst (LPA), Jaime Snow. The facility file was reviewed prior to this visit. A review of the personnel report on file indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. This is a combination center with separate preschool, infant and school-age licenses. The facility’s operating hours are 6:30 am - 5:30 pm, Monday - Friday. The facility was toured inside and outside, and the floor and yard plan submitted by the licensee were verified. Activity space for each age group is physically separate from other age groups. The items which could pose a danger to children (such as detergents, cleaning compounds and medications) were observed to be inaccessible to children. The director stated that there are no poisons in the facility, and none were observed during today's inspection. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces are clean, toxic free, safe and in good condition. There is uncontaminated drinking water available to children both indoors and outdoors via individual water bottles. The children's bathrooms are in safe and sanitary condition. A current menu was posted by the entrance. Food prep areas are clean. Food is properly stored and refrigerated as needed. Garbage cans containing solid waste have tight fitting lids. The playground was free of hazards. The playground equipment and surface areas are in safe condition. There is wood chip cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed. The director stated no weapons are stored on site and none were observed. The large play structure had exposed metal in at least 3 places that was too hot for the LPA to touch at approximately noon 96 degrees Fahrenheit. There is also a large PVC pipe surrounding the sandbox; children must stratal it in order to enter the sandbox and it was too hot for the LPA to place a hand on for more than 2 seconds. During today's inspection, staffing ratios were being met and 30 being supervised by 7 staff. The facility was operating within the licensed capacity. At least one staff member present during the visit possessed current CPR and First Aid certifications. Six children’s records were reviewed at 1:15 pm, and contained identification forms with authorized representative information. Six staff records were reviewed at 1:20 pm, and contained documentation of education and training, as required. This facility is not providing Incidental Medical Services (IMS). The Department’s Incidental Medical Services (IMS) policy was discussed with the director.     Outdoor Activity Space. The surface of the outdoor activity space shall be maintained free of hazards.There is exposed metal on the large play structure. The regulation has not been met because there is exposed metal on the large climbing structure that is too hot for the LPA to touch with bare hands (the outdoor temp is 96 degrees F today). There is also a hot PVC pipe surrounding the sand box. As evedenced by the LPA's observations.
