What the inspector wrote

On 2/2/2024 at 12:20 p.m.,Licensing Program Analyst LPA J. Gifford conducted an unannounced complaint inspection for the purpose of delivering complaint findings and met with licensee Susan Wilson. It has been alleged that staff are not conducting 15-minute Safe Sleep checks. On 12/20/2023 at 2:07 p.m. licensee stated, the 15-minute Safe Sleep checks are being conducted. The licensee stated, staff complete the 15-minute Safe Sleep checks in books located in the 2 infant rooms. Each staff member fills out an infant sheet with the check marks and their initials. LPA, J. Gifford interviewed six staff (S1-S6) on 12/20/2023 between 12:20 p.m., and 2:07 p.m., (S7-S8) on 12/22/2023 between 10:14 a.m., and 11:05 a.m., and (S3) again on 1/11/2024 between 12:29 p.m., and 12:38 p.m. and (S2) on 1/12/2024 between 12:39 p.m., and 1:08 p.m. Seven out of eight staff members denied the allegations of staff not conducting 15-minute Safe Sleep checks while the infants are sleeping. (S1-S4), (S6-S8) all stated the 15-minute safe sleep checks are being done and the staff are writing in the book and on sheets of papers to provide the parents with. (S5) stated, sometimes I am not sure if the 15-minute Safe Sleep checks are being done. (S5) admitted, to not always completing the checks every 15 minutes. LPA J. Gifford interviewed eight parents (P1-P8) on 12/22/2023 between 10:40 a.m., and 1:25 p.m. Eight out of eight parents denied that staff are not conducting the 15-minute Safe Sleep checks. All parents stated they have observed staff conducting checks on the infants and staff providing parents with infant papers which indicate they have been checked. On 1/17/2024 after reviewing 22 Safe Sleep 15-minute check records, LPA, J. Gifford observed check marks every 15 minutes for each infant. Out of the 22 sleep logs reviewed, 7 sleep logs were observed to be missing initials or signatures signing off the checks had been completed. Although there is evidence that the required 15-minute (Safe Sleep) checks were being completed, not all the required information was documented and logged each time a 15- minute (Safe Sleep) check was conducted on a sleeping infant by facility staff. Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. Exit interview was conducted and report was reviewed with the licensee, Susan Wilson. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
