What the inspector wrote

According to CD, the transition policy consisted of staff conducting a name to face check which meant staff were required to stand at the classroom doorway threshold and have one foot in the room and one foot in the hallway. Staff confirmed that part of the transition policy included reciting and document the children's names on the Children's Supervision Record (CSR) while staff watched and verified each child transition through the door. Part of the process required staff transitioned the children out of the class into the hall, and from the hall to the designated classroom or area while repeating the name to face process. Additionally, staff count the number of children and check marks which should match. The restroom policy was for the kids to utilize the restroom at their leisure, as long as no one else was in the restroom. Furthermore, CD reported an incident involving a child (C1) being left alone in a classroom for about one to two minutes, and staff admitted they did not follow protocol for the transition policy; and they did not correctly conduct the name to face checks while the children transition; resulting in C1 being left alone in the preschool classroom. AD said she observed S2 entering the pre-k classroom and while AD was attending to other matters in the preschool class, AD did not see or hear anyone in the classroom, but AD acknowledged that she did not look in the bathroom. The pre-k classroom was in ratio right after the transition, so S1 signed out and left the pre-k class to find a parent in the preschool classroom which was talking with S3 in the toddler class that was positioned at the doorway. The statement C1 and P1 provided did not report any concerns related to violation of care and supervision. C1 validated feeling safe at preschool, while P1 expressed appreciation of staff transparency in immediately notifying parents of incident(s) and what steps the facility would take to ensure that incident did not re-occur. Furthermore, LPA obtained CSR which documented children's transition and staff time in/out which showed C1 was alone in the restroom for approximately one minutes. Based on the investigation, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be substantiated. The following violation of the Health and Safety Code is being issued: see LIC 9099D. Exit interview was conducted, and report reviewed with Center Director Erika Silva. Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided.
